School Health Forms
Standardized School Health Forms
The Department of Education has worked jointly with the Department of Health and Hospitals-Office of Public Health and the School Health Advisory Committee that was established by Act 685 in the 2004 Regular Legislative Session to develop school health forms (see attached). The use of the forms will have an overall positive impact on the health of school aged children in that the forms will ensure uniformity in the documentation of medical information submitted to schools and school nurses.
The standardized school health forms are designed to eliminate the duplication of information submitted to schools and school nurses relative to health information and screenings, allergies, illnesses, sports physicals, medication administration, and prescribed procedures.
R.S.40:5.12. requires the use of standardized health forms to be used exclusively in all city and parish public school systems by August 2007.
