Errors on Transcripts

The Family Educational Rights to Privacy Act (FERPA) allows an educational agency to amend an education record if the record contains information that is inaccurate, misleading or an invasion of privacy. However, the Department of Education does not have the authority to change the information on documents submitted from local education agencies. This authority is granted to the local school system generating the high school transcript. Please contact the local school system from which you graduated for policies related to amending educational records.


Copies of High School Transcripts and Diplomas

Permanent records are under the control and care of local schools and school systems. Records maintained by the Department of Education are copies submitted by the local schools and school systems.

If you graduated from one of the following school districts, you may contact them directly to obtain a duplicate transcript or reissued diploma.

Requests from the State

To request a copy of a high school transcript or reissued high school diploma, please the steps below. Because the volume of requests is unpredictable, we cannot guarantee the amount of time required for processing.

1. Complete the request form and mail to the address on the form. Be sure to include the correct year of graduation and high school. If the student received a GED, please contact the Division of Family, Career and Technical Education at (225) 342-0444.

2. Because these records are confidential, the graduate must authorize their release. If a third-party individual or agency is making the request on behalf of the graduate, the request must include a signed release statement from the graduate and a copy of the graduate’s driver’s license or other state-issued source of identification.

3. Include the appropriate fees. There is a non-refundable fee of $2 per transcript and $10 per diploma. We can accept money orders, cashier checks, and company checks. Fees are nonrefundable.

Important Notice: Students graduating from nonpublic schools that did not complete the state minimum requirements for graduation are not eligible for a reissued state diploma. These students will, however, be provided with a copy of the transcript submitted indicating their graduation status from the previously SBESE approved nonpublic school.

4. Because a reissued diploma is not valid until it is signed by the current local Superintendent, the School Board President, and by the local high school Principal, more processing time is required. The Department of Education will mail the diploma directly to the local school system, which will obtain all necessary signatures and then forward it to the graduate.

High School Graduation Verification Service

In accordance with the Family Educational Rights to Privacy Act (FERPA), certain information is deemed public, and schools are allowed to disclose those records, without consent, to the following parties or under the following conditions (34 CFR § 99.31):

School officials with legitimate educational interest.
Other schools to which a student is transferring.
Specified officials for audit or evaluation purposes.
Appropriate parties in connection with financial aid to a student.
Organizations conducting certain studies for or on behalf of the school.
Accrediting organizations.
To comply with a judicial order or lawfully issued subpoena.
Appropriate officials in cases of health and safety emergencies.
State and local authorities, within a juvenile justice system, pursuant to specific state law.

Please note that generally, schools must have written permission from the parent or eligible student in order to release any information from a student's education record. However, "directory" information such as a student's name, date of birth, honors and awards, or date and place of graduation may be released without consent.

Parents and eligible students may request that directory information contained on high school transcripts not be disclosed from our office by submitting a written statement of nondisclosure. The signed statement will be kept on file.
