Professional Services Vendor Billing Form

All professional service contractors are required to submit an invoice via the following Contract Billing Form to their respective program negotiator. Usage of this form enables the Department to collect all required information necessary to issue payment.

All invoices for professional service contracts ending June 30 or earlier must be submitted no later than July 15 annually.
